A living will is a document in which a competent adult states in writing that he or she does not wish to have his or her life artificially prolonged by modern medical techniques.
In 1991, the Ohio legislature passed a statute authorizing Living Wills which specified the exact language and formalities required.

| The declarant must be at least 18 years old. | |
| The declarant must be competent. | |
| The purported document must conform to Ohio law. | |
| The document must be either witnessed before two impartial persons or acknowledged before a notary public. |
